Consultation requests
The contact form asks for your name, email address, an optional phone number, who the request is for, an optional preferred format and time to reach you, and a short message. It sends those details through this website’s server to Formspree, the practice’s form service, for processing and delivery to Erika. You can read Formspree’s privacy policy (opens in a new tab) for information about its handling of submissions.
Please do not include medical history, diagnoses or other sensitive clinical information. Using the form or email does not establish a therapeutic relationship or confirm an appointment. Contact Erika to discuss a suitable way to share clinical information.
Technical information
This website is hosted on Vercel. Hosting and form providers may process technical information, such as IP addresses and request information, to operate and protect their services. The site does not include advertising pixels or session replay. Application code does not log the text of your inquiry.
The site does not use visitor analytics tools.
Phone, email and external links
Phone and email links open your chosen application. Messages you send that way are handled by your phone or email provider and the recipient’s provider. External websites have their own privacy practices.
